For adoption records, please visit our Adopted Through DePelchin page.
For confidentiality purposes, only clients may request or authorize the release of their records. If the client is under the age of 18, the parent, conservator, or legal guardian may request or authorize the release of records. Please note there is a fee for copying records.

Please fill out the form completely:
- Each field on the form must be completed.
- Be sure to submit a complete address and telephone number for the person to whom the information should be released.
- If the item needed or purpose for the release is not listed, please write it on the line marked “other”.
- Initial the box where appropriate.
- Sign and date the bottom of the authorization.
